Registration

Please complete one registration application per child. A registration fee of $100 per child is due at the time of enrollment. No registration fee is required for our drop-in program. An updated immunization record signed by physician is due on your child’s first day. Individual Health Care Plan form signed by physician (if applicable for your child).
